Image rights authorization form
According to GDPR, the Geneva LIPAD, RIPAD, and the University Directive, we need to collect your authorization to collect and publish images of you.
Please download, fill and return us(it-cisa[@]unige.ch) the following form:
I, the undersigned XXX, declare that I authorize the Centre Interfacultaire en Sciences Affectives (hereinafter CISA) :
- to use the photograph(s) and/or video and/or audio recordings of me taken in the premises of CISA and/or in the various events under its responsibility (workshop, conferences, etc.).
- to use them, on paper and/or electronically, as part of its communication, for example social networks for which the CISA is responsible and on the CISA website, for different audiences, without geographical or time limitation.
- to use the photograph(s) and/or video and/or audio recording(s) of me that I provide the CISA with for its communication on the CISA website.
The delivery, the shooting or sound recording and the display of these photos and/or recordings are granted free of charge. I have taken note and accept that a change in the images and/or recording (framing, color, density, etc.) may be carried out by CISA, its representatives or any person acting with its permission during the reproduction of the photos and/or recording. These changes will not affect my integrity.
I have taken note of and accept that the University of Geneva declines all responsibility for the use by third parties of the images distributed (copies, plagiarism, etc.). The University of Geneva nevertheless undertakes to inform me of any illicit use of the images that may be brought to its attention.
I have taken note of the fact that the University of Geneva undertakes not to use the photos and/or recordings representing me for purposes other than those described in the present attestation without my prior agreement and that I may revoke my authorization for the future at any time.
I declare that I am 18 years of age or older and have full civil rights and have read, understood and accepted this authorization.
